	If you would like Perfect Gift Puzzles to make a personalized jigsaw puzzle for you from a photograph taken by a professional photographer, you must first get permission from that photographer. You can do this by printing the form below and taking it to the photographer. We will make no puzzles from copyrighted photos without the consent of the photographer.
